IMPORTANT HEALTH COVERAGE TAX DOCUMENTS
Effective January 1, 2019, Congress eliminated the individual penalty for failure to maintain health insurance coverage. As such, the IRS has determined that the Form 1095-B no longer needs to be distributed to Health Plan participants. The Form 1095-B is not required as part of your annual tax filing. In accordance with these guidelines, you will not receive a Form 1095-B from the Health Plan for the 2025 tax year unless you request a copy.
The Form 1095-B is now available, by request only. MyPHP subscribers who request a digital copy of their Form 1095-B can have it delivered electronically to their myPHP portal. If you have not subscribed to myPHP, visit dgaplans.org/about-myPHP for more information about the many benefits available to myPHP subscribers, including up-to-date, personalized information about your pension and health benefits status.
