Employer Contributions Forms

The following forms are for signatory production companies making contributions to the Directors Guild of America – Producer Pension and Health Plans. The forms are available in both Microsoft Excel format* and Adobe Acrobat format (click here to download the free Adobe Acrobat Reader). The appropriate form is based on the agreement under which the reported project is being produced. You may report multiple projects from the same agreement on the same form.

About Us

Created as a result of the Directors Guild of America's collective bargaining agreements with producer associations representing the motion picture, television and commercial production industries, the DGA-Producer Pension and Health Plans provide excellent benefits to participants.

The DGA-Producer Pension and Health Plans are separate entities from the DGA and are administered by a Board of Trustees made up of DGA representatives and Producers' representatives.
