POTENTIAL PENSION CHANGES

  • If your Pension Plans assets are a subject of your divorce proceedings, please contact the Pension Plan by phone at (877) 866-2200, Ext. 404 or by email at pension@dgaplans.org. We can assist you in gathering the information that you need.
  • If your situation requires the filing of a Qualified Domestic Relations Order (QDRO), the Plan Office can provide you or your attorney with a QDRO kit. This kit includes the Pension Plan’s QDRO procedures and model QDROs for both pension plans.

DOCUMENTS TO UPDATE

  • Beneficiary Designation Form – after a divorce you may want to change your beneficiary designation, which requires you complete a new Beneficiary Designation Form.
    • You must file a new Beneficiary Designation Form to change your beneficiary.
    • Once the Pension Plans receives your new Beneficiary Designation Form, all previously submitted beneficiary designations become invalid.
  • Address Change Form – if you or your ex-spouse moves, it is important to update your address with the Pension Plans.
  • Revocation of Third Party Authorization – if you previously completed a Third Party Authorization Form authorizing the Pension Plans to speak with your spouse about your benefit accumulations, eligibility or any pension-related matters and would like to revoke that authorization, you will need to provide written notification to the Pension Plans.

  • You must notify the Health Plan within 60 days of your divorce. At this time a copy of your divorce decree must be provided to the Health Plan.
  • After a divorce, your ex-spouse no longer qualifies as an eligible dependent under your Health Plan coverage.
  • Your ex-spouse is eligible for 36 months of COBRA continuation coverage, beginning on the date of divorce.
  • To enroll in COBRA coverage, your ex-spouse must contact a participant services representative at (877) 866-2200 Ext. 401 within 60 days of the divorce decree.
