Moving can be hectic, which sometimes means important things are lost or left behind. When preparing to move, make sure to let the DGA-Producer Pension and Health Plans know your change of address so that any important health and pension related information follows you.  

COMPLETE AND SUBMIT A CHANGE OF ADDRESS FORM

Ensure your Plans related mail is sent to the right address by keeping your contact information up to date, using any of the following methods:

CALL THE DEMOGRAPHICS DEPARTMENT at (323) 866-2200, Ext. 407 to update our contact information over the phone.

CHANGE YOUR ADDRESS INSTANTLY ON myPHP

  1. Create a myPHP account or log in here.
  2. Go to My Profile
  3. Click Edit Addresses

ADDITIONAL CONSIDERATIONS

The DGA-Producer Pension and Health Plans and Directors Guild of America are separate entities. You must notify the DGA separately of your address change. Their contact information is 7920 Sunset Blvd., Los Angeles, CA 90046; phone (310) 289-2000 / (800) 421-4173.
