Affordable Care Act Information for Participants
The DGA–Producer Health Plan is providing the information below to assist Health Plan participants with certain aspects of the Patient Protection and Affordable Care Act (“ACA”).
Frequently Asked Questions: ACA-Required Forms 1095-A, -B and -C

If you were covered by the DGA–Producer Health Plan during 2018, the Health Plan will send you a Form 1095-B. Form 1095-B is the only ACA-required form that the Health Plan must provide to Health Plan participants.
The other forms, Form 1095-A and 1095-C are sent by Health Insurance Marketplace insurers and certain employers, respectively.
For questions about Form 1095-A, contact any Health Insurance Marketplace insurer with which you had coverage during 2018.
For questions about Form 1095-C, contact your employer(s).
For 2018 tax year, the deadline to mail the forms is March 4, 2019.
If you were covered by the DGA–Producer Health Plan during 2018, the Health Plan must mail Form 1095-B by the deadline above.
Form 1095-B will identify which months during the past year you and your dependents had coverage with the DGA–Producer Health Plan.
You may receive additional Forms 1095-B from other insurers if you had other non-Marketplace coverage during the past year.
Keep the form for your tax records. You don’t actually need the form in order to file your taxes, but when you do file, you’ll have to tell the IRS whether or not you had health insurance for each month of 2018. Since you don’t actually need this form to file your taxes, you don’t have to wait to receive it if you already know what months you did or didn’t have health insurance in 2018. When you do get the form, keep it with your other 2018 tax information in case you should need it in the future to help you prove you had health insurance.

The Affordable Care Act was in effect for the entire year of 2018. IRS tax forms still require you to report whether or not you had health coverage in 2018. The penalty for not having health insurance after January 1, 2019 has been reduced to $0, but it still applies for the months of January through December 2018.
No. Although a judge in the Northern District of Texas issued an opinion in December 2018 that the ACA is unconstitutional and invalid, this decision has been appealed. The ACA remains in place and will be enforced during the appeals process.
Possibly. It depends on whether the federal government will require us to report health coverage for 2019. You shouldn’t have to pay an IRS penalty, though.
If you were covered by the DGA-Producer Health Plan during the past year and have not received a Form 1095-B from the Health Plan by mid-March(accounting for postage time), call Participant Services at (877) 866-2200 ext. 401 or email us at ACAQuestions@dgaplans.org to request another copy.
If you do not receive a Form 1095-A from a Marketplace insurance provider with whom you had coverage during the past year, you will need to contact that insurer directly.
If you do not receive a Form 1095-C from an employer and believe you should, you will need to contact that employer directly.
