Dependent Audit Closed

Effective Thursday, October 20, 2011, the Health Plan’s dependent audit is closed and coverage will be terminated for all unverified dependents.

For those participants whose dependents are losing coverage as a result of misplaced birth and/or marriage certificates:

Please contact your state’s Vital Records office and request a replacement copy of the misplaced birth or marriage certificate. Once you have done so, please submit a copy of your request to the Health Plan’s Eligibility Department at:
